A Trial of Drug Use Evaluation on Amikacin in Pediatric Patients






Abstract
A retrospective drug use evaluation(DUE) study was conducted on amikacin by reviewing pediatric patient¡¯s charts. "Criteria for Drug Use Evaluation" estabilished by Amer. J. Hosp. Pharm. Was selected, reviewed and utilized in DUE process. Total 100 charts of patients who treated with amikacin in Seoul National University children¡¯s Hospital from Jul. 1990 to Jun. 1991 were retrospectively reviewed. As a result, 30 cases were met with the criteria for the justification of use, while remaining 70 cases were deviated from the criteria. In analyzing process indicators, several criteria including body temperature monitoring, WBC monitoring, appropriate initial dosage and duration of therapy were relatively well documented showing the accepted levels above 75%, while serum creatinine monitoring, culture & sensitivity test and serum drug level monitoring were infrequently documented with low accepted level. In outcome analysis, blood cultures after discontinuing drug were rarely documented with accepted level of 20%. Considering that the DUE program provides a method for defining, assessing and improving the quality of drug use, it is suggested that the above findings should be communicated to the medical staff, and a active intervention, such as feedback control, also be necessary for rational drug use.
